GUI layout not working

I have a gui that is not formatting the way I want it to. It should look like this:

I bet it has something to do with the layout :S

You'll want to post your pertinent code here in the forum since we're all volunteers and quite busy with our families, jobs, lives, etc. I assume you've read the tutorial on how to use Swing layouts, right? Do you have any specific questions regarding the tutorial?
