Don't get info from form into mail

Hello,

I am using the form which you can find here . But when I fill in the form and I submit (on the left bottom of the page) I don't get the data from the form into the mail. :confused:
I am a just a beginner and used a standard script which I expanded.

Can anyone help?

Are you talking about the formatting into the mail?

When I hit send my Office Outlook appears with a new email address and a horrible looking format of information.

on a side note(Java and Javascript are very different)
