How to prepare Server Document.

Can any one help me in finding out the questions for the documentation on server.

Actually my client does'nt have any documentation on servers, so i a meeting my lead asked me to prepare a documentation for the servers, which we are using so that they can keep in a website which is accesable for every one in future.

So my question is what kind of info do i need to collect on the servers to prepare document on it.

The following info:
- What are the servers you care about?
- For each such server:
- why are you using it?
- what does it do?
- how to connect to it?
- what's the API?
- who to contact with additional questions?

would be a start...
