    Default Forms Question

I am trying to make my own form like on this site below. I want to be able to have users enter data in like 5 fields, and press calculate and get a number returned.

Get a number based on what? You should be much more specific. This post is like I told you: my car won't start, what's the problem?

Seems that bradk8605 missing his link in the post. Actually bradk8605 you can't add links at the beginning. Simply paste the text here in your post and send it to here.

Or else, explain your question well. No one haven't an idea what you really want.
