JAVASCRIPT code for button

Hello,
I'm new to JAVA programming. I want a javascript code for button. The button must be like this


When we click on a Button(with some name) it must open a text box window like this

Text line 1 to left (with color)

Text line 2 in middle within BOX ( with some color)

Text line 3 (with color)

Text line 4 (with color)

Text line 5 (with color)

Bottom right close button

Any number of buttons with corresponding text boxes must be supported within the same code
